Pooled Trust 1 Corpus
The following documents pertain to the CLC Foundation Pooled Trust 1.  If the trust is being funded by a relative, friend, or guardian, then the following documents are necessary.  If the trust is being funded by the beneficiary themselves, then please refer to the documents listed under Forms Pooled Trust 2 Documents for Pooled Trust 2. 

The Pooled Trust 1 Corpus tells you about the trust itself.  The Sponsorship Agreement Pooled Trust 1 is what "joins" you to the trust.  This agreement is also known as the joinder agreement. This is the document that needs to be filled out in order to join the trust.  A check should be made out to Wachovia PT 1 FBO (persons name), for the amount that is going to fund the trust account, and for every check written on behalf of the beneficiary that is to be deposited into the trust in the future.

The amendment to the Corpus is necessary to note the changes that have been made regarding both the Trustee as well as the Fiduciary.  The Trustee for the Supplemental Needs Pooled Trusts is John J. Signorelli, who is also the Executive Director of the CLC Foundation, Inc.  The Fiduciary of the Supplemental Needs Pooled Trust is Wachovia Bank.  It is no longer Merrill Lynch.  The CLC Supplemental Needs Pooled Trusts are managed by the CLC Foundation, Inc.
